During employment through this agency, I was staffed at "Jensen Concrete" in

Casa Grande, Arizona, and "Arizona Nutritional Services" in Tempe, Arizona.

• At Jensen Concrete, I was the helper/laborer for their company Foreman. Some of my

responsibilities involved thoroughly cleaning forms, chalking forms, preparing those forms to specific

specifications. In the course of these task, I applied basic arithmetic using a tape measure, table saw, and

misc. hand tools to ensure quality products were produced.

Some other responsibilities included cleaning of work spaces, bending re-bar, tying re-bar

cages together for production, and assisting a crew load the various concrete products onto semi-trucks.

• At Arizona Nutritional Services, I was responsible for working directly on the production floor loading and

filling shipping orders on the production lines. After each pallet/orders were complete, I had to ensure each

pallet was stacked properly and efficiently, and all labels were correct per work orders; after which, all pallets

were wrapped properly to the standards of A.N.S. Q&A.

Laborer/Production

Adecco Staffing Agency - Casa Grande, AZ - 2012 to 2012

While working through Adecco, I provided the opportunity to work at "Bright Group International". Most of the

duties/responsibilities I had at Bright were checking the qualities of Redken, L'Oreal, and other hair/bleach kits

before loading them into a shipper and place them on a pallet in a specific layout. I was also in charge of the

wrapping of each pallet and loading them onto the stationing floor in chronological order for the forklift drivers

to pick them up and place them into the shipping bay.

• I occasionally was placed in the maintenance department to help clean the production lines. (Prevention of

Cross-Contamination)

Lead Prep/Day Shift

Papa Murphy's Take & Bake Pizza - Casa Grande, AZ - 2011 to 2012

Opening store, stocking lines w/ needed food items and condiments. Preparing dough, and prepping

vegetables Operated (PLU) register, handled money, and provided great customer service. I was in charge of

checking inventory orders, making sure order was correct and all products were received Upon order intakes,

stocking them in the store, and rotating the current inventory.

K & B Landscaping - Chesapeake, VA - 2011 to 2011

Worked as crew supervisor, and hedge men Other job requirements included: mowing, weed whacking, lawn

aeration, tilling land, trimming hedges, and general property maintenance. I worked with Stihl hedge trimmers,

Toro and Exmark "ZeroDegree" mowers.( Deck sizes 36" - 72". If it cut grass, I learned to use it at this company.)

J.W. Combs Electrical Contractors - Portsmouth, VA - 2010 to 2011

Helped Foreman and Journeymen in every way required. Bending, measuring, cutting EMT pipes.

Run conduits through walls, ceilings, and crawl spaces. Pull wire, through walls and or ceilings. Hook up

different sorts of lighting, power supplies, and receptacles. Kind of learned the basics of reading blueprints.

(Also, spent some time helping the Brick Masons with cutting CMU blocks, and other

needs to help speed up process the electricians putting boxes and piping in walls, while I did my own required

duties for J.W. Electric.)

Sun Lakes H.O.A #2 - Chandler, AZ - 2003 to 2007

I started working at Sun Lakes H.O.A. #2 as a busser/expo. After I turned 19, I was legally allowed to serve and

bar-back. As a busser/expo, some of my duties were setting up restaurant tables with proper utensils, glasses,

cups, and napkins. I was also responsible for keeping the customers tables cleaned, and providing help to the

servers by getting bread, water, salads, and appetizers. As an expo, I had to place the proper garnishes on

each plate and then take the orders to the customers.
