and symptom management as well as attend to the educational needs
identified by contracted nursing facilities.
Provide Palliative Care consultation for care needs of patients within
community, home, nursing homes, assisted living centers. Triaging acute
symptom management for VNA/Home care patients who present with Non-cancer
diagnosis, Cancer, and Chronic pain issues. Coordinate care with clinicians
when plan of care changes from acute to palliative, easing transition
towards care and comfort.

Position involves assessing/triaging the Psychiatric needs of hospitalized
patients within Med/Surg, ICU, and Emergency department.
