Qualifications: I have 23 years of extensive experience in all phases of Level 1 Trauma Radiography, as well as: Surgery, Nursery, ICU, Pain Management, Cadaver Lab, Portable and C-arm imaging. I have applications grade knowledge of the OEC 9900-9800 C-arms. I am rock solid in Surgery, Trauma, Pain Management and Portable Imaging. I am also registered in Mammography and am cross trained in Computed Tomography. I have worked with a huge variety of X ray equipment, Film Screen, CR and Digital systems as well as several different versions of PACS systems. I have excellent self starting capabilities. I am meticulous in my imaging and take great pride in my work. I require no supervision once oriented.

Should I be considered for a position at your facility. I will consider driving up to 100 miles one way. I also have a self contained 35' RV which if necessary I can bring to your facility if call is required.

I was hired by UHG operate their C-arm for Pain Management cases.

My duties include: explaining procedure to and positioning of the Patient, correct positioning of the C-arm, sterile tray set up, prepping and draping anatomy using sterile technique, preparing medications for procedure, transporting patient to recovery room and procedure room turn over. I have also become the Director of Imaging in charge of inventory and ordering of all diagnostic drugs, equipment and supplies. As well as Radiation Safety and Monitoring of Employees. I am an integral part of the Pain Team here as I have introduced several new injections learned in Cadaver labs. I will also be a key person in the Pain Management Suite remodel which is scheduled for the very near future.

As a LUH Radiologic Technologist on the 2nd shift I was responsible for all procedures house wide. This included all In Patient and Out Patient exams. The main core of my daily duties revolved between the Emergency Room, Surgery (after hours Orthopedic and Cysto Procedures) and Day Surgery procedures such as Pain Management and Pacemakers. I was utilized as a Resource (Go To) Technologist - by the Lead and my Co-workers. I was also a PACS Super User. I have applications grade knowledge working with OEC 9900 and 9800 C-arms. I am proficient at sterile tray set up. I am rock solid in Trauma, Operating Room as well as, any and all Portable exams.

The last phone number I have for Dr. Landis is 970-***-**** it's my understanding that he has retired.

I primarily followed Dr. Valdon Landis to the MVSC and NCSC facilities to do Pain Management cases.

This included explaining procedure to and positioning of the Patient, correct positioning

of the C-arm, sterile tray set up, occasionally prepping and draping anatomy using sterile

technique, preparing medications for procedure, transporting patient to recovery room

and procedure room turn over.

Some of the exams preformed on a regular basis were:

Cervical, Thoracic, Lumbar and Caudal ESI's. Facet Joint, Intercostal, Hardware,

Pariformis, Sacral, Selective Nerve Root and SI Joint Injections. Radiofrequency exams

and on a fairly regular basis Sympathetic injections.

I also worked Perdium at the Loveland Surgery Center, covering the pregnancy leave of

their Pain Management Technologist which primarily focused on Spine Surgeries and

Pain Cases.

Through this experience I have become well versed in a multitude Pain Procedures as well as Radio Frequency Procedures, with a number of different Physicians.

I worked extensively with the OEC 9900 and 9800 C-arms.
