Greetings to my prospective employer. My name is Michelle West and I am looking for full time employment with your company. I have been an Licensed Practical Nurse for the past 21 yrs since graduating from CloverPark Tech College in 1991. I have also worked in various fields in the medical arena including Long Term Care, US Army, Chemical Dependency, Corrections, and Pediatric Home Health. I have been certified in the past for IV, Central and PICC Line care and access. In addition to holding a license in Washington State, I have also held licenses in North Carolina and Tennessee. For the past year I have taken a leave of absence to help care for both my father and step-father, who were both diagnosed with cancer. Compliant with Medicare/HMO charting regulations

Able to complete incidental charting as required in a ledgible handwritting

Able to complete patient/employee incident reports accurately

Able to transcribe/verify physician orders (written, verbal, telephone) in an accurate/competent manner

Able to follow written/verbal orders from RN/DNS

Able to resolve customer concerns quickly without the situation escalating

Able to order, recieve, login, logout and destroy medications including narcotics per facility/FDA policy

Able to inform primary/attending physicians of critcal issues with competence

Albe to perform venipunture with competency using vacutainer/butterfly via peripheral vein, IV, Central Line or PICC access

Able to perform Central Line/PICC Line care using sterile technique

Able to perform IV start/removal in a competent manner

Able to infuse IV fluids and medications per physician/pharmacy orders

Able to perform naso/pharengeal, trachial, and oral suctioning with competence

Able to insert/remove urinary catheters from male/female and urostomy using sterile/clean technique with competence

Able to insert/remove gastric feeding tubes per physician order as needed for dislodgement/replacement

Able to start/stop nutritional feeding, assess abdomin, and instill medications via gastric feeding tube with competence

Able to perform physical assessment with competence

Able to perform SQ/ID/IM injections with competence

Able to apply topical, oral, vaginal, and rectal medications with competence
