Work site was at the Tuck Power Plant near Texarkana; AR. Heavy dirt work with mobile equipment was underway. Work included retention ponds (several) with liners and a landfill location that was lined. Rail road berms were built for inbound trains that would deliver coal to the site. A daily Safety meeting was held to keep the employees aware of safety and upcoming changes in the work routine. Observed inspection and startup of equipment daily. Observations were made of the work locations throughout the day. Assistance to the employees was provided when directed or asked for by co-workers. Maintained computer communications for the manager as needed. Daily reports were maintained and turned into the General Contractors office as directed. Would have provided First Aid as needed but there were no incidents.

Reason for leaving: I was laid off by the General Contractor as they were paying for my activity.

The work site was at the East Waste Water Plant, San Antonio, TX. The worksite was a Solar Power Farm for the City of San Antonio, TX. Heavy dirt work was completed to allow for the clearing of the site to a depth of 18", leveling, and building of roads. The work was in two zones separated by the Plant. A work group was allocated to each zone. Daily Safety Briefings were completed prior to work. Observations were made of the Mobile Equipment inspections, fueling and startup. The two work zones were observed throughout the day as work was completed. When necessary I would complete the Supervisors briefing at the start of day. By request or under the direction of the Supervisor I would assist with getting supplies as necessary for the site. Inspection of PPE was done in the morning and during the work zone surveys. PPE was provided as necessary.

Site Safety Manager

The work site was a Military Reserved Center in Jonesboro; AR. Initial work was draining the property and removal of spoil from the site. Assisted in the pumping operations and filtration that was required. I went off site to gain materials that would assist in the filtration process. Prior to work all work groups had to provide a Job Safety Analysis was to be presented for review. If not complete assistance was provided for completion. As mobile equipment came to the site it was inspected prior to use. Supported the guide of not storing motor fuel on site due to the drainage to irrigation ditching surrounding the property. As tool boxes were brought on site the tools were inspected to insure they were secure and power cords met the double insulated rule. Daily observations were made of the equipment inspections and startup. If there was an issue with a piece of equipment it would be put out of service and made the responsibility of the Sub Contractor on site. Weekly managers' meetings were attended and information regarding Safety was presented. Any new work startup was brought to the attention of all. Safety Meetings were held with the employees of the Sub Contractors once weekly. PPE was the starting topic for each meeting to insure no one would forget. This included the use of fall protection as required. The topic use of PPE for such as welding, cutting, pipe threading, gloves, footwear, hearing was brought out. When an employee was not using PPE this would be brought to the attention of the supervisor for him to manage. When needed I would assist any employee as requested or if directed by my Superintendent. Scaffolds were inspected daily to insure they were secure and the tag would be signed. Daily reports were completed with copies sent to management.

Reason for leaving: A senior Safety person came to the site, I was laid off, last in first out.

Safety Consultant

The site was Bass Hall at the University of Texas, Austin, TX. The building was a stage theater nine floors tall. It was a heavy demolition and remodel interior and exterior. As each sub-contractor came to the site they were provided with an orientation for the site. All mobile equipment and tools were inspected prior to work. As work was started it was observed several times daily by walking the building and observing work underway. If an unsafe act was observed the employee would be warned and the supervisor contacted. Lifts used outside were inspected daily and employees reminded to wear a body harness secured to the lift. A large scaffold was erected on the inside of the theater that extended work to the interior ceiling of the building. Work required the use of fall protection which was inspected. The scaffold was inspected prior to use daily and the inspection card signed. Fire watches were required due to hot work above the seated area and in the orchestra elevator pit. Fall protection was required for different groups as there were large openings in the side of the build. First aid was provided to anyone asking for assistance. A weekly safety meeting was held with the Lead man from each group discussing any observations made. If a new type of work was forthcoming they would be advised. A daily report was developed each day noting the work groups, zone of work and use of PPE as required.

Reason for leaving: Management refused to enforce safety in IDLH exposures. I went to the owners, University of Texas, who in turn went to constructor group. I was terminated.

For people I knew I would assist them on processing property and auto claims in a timely manner with their carrier or a third party carrier.

For my employer I was stationed at remote construction sites and buildings that needed night time security. The construction sites were for a large highway construction that were in the Austin, TX area, offices, and heavy equipment operations with large stores of base materials for use. Observations were made protecting the sights from possible theft or vandalism by unwanted persons. If any incident occurred or was possible management would be called promptly. All work was completed during the darkened hours.

As a sub contract employee I worked for Samsung at the second addition to the plant. This was a high paced site that went to 24hours a day operation. Each day I would observe the different work groups start the day to insure the all had PPE as needed for the site. Fall protection was observed on the employees to insure it was secure. If an unsafe act or condition were to occur a supervisor would be contacted quickly and the problem resolved. It was insured that each employee was provided with PPE each day. Hazardous materials were observed to insure they were secured in containers in a safe and identified location. As the building went up and was dried in for painting I was laid off as my portion of the work was completed.

For my employer I cut glass to the customer needs and installed the glass in business operations and private residences. Custom glass enclosed showers were developed for customers and when receiving the glass it would be installed in the residence as requested. Bids were developed for custom glass at businesses and residences. Double pane glass was replaced in residential homes as the seals gave way causing the window to fog.

Work spanned from Georgetown, to San Antonio, TX with the general office in Austin, TX. The operations were a landfill site, motor fleet operations in Georgetown, Austin, and San Antonio for commercial and residential disposal services. Several locations in the area were retail sales outlets for organic products. Riding with the fleet operations I could see hazards that could be resolved and action could be taken. Observing the landfill operations I saw how they routed traffic to reduce the exposure to vehicle and possible foot traffic. The site and equipment was inspected for safety items that could be resolved. Monthly meetings were held at the different fleet locations to keep the drivers up to date on activities or hazards on the IH 35 corridor. Shop employees were provided with a safety meeting monthly to discuss housekeeping and working with heavy objects. Lock out/Tag out was always discussed due to the hydraulics on all the transport vehicles. All incidents of workers compensation, back to work, auto liability, and property liability was managed with detail files kept. Directed all claims to conclusion.

Reason for leaving: A preventable fatality occurred that management retreated from.

Laredo Paving, Big Spring, TX

The company was a highway paving contractor with a general office in Big Spring, TX. The general location had a heavy truck shop, paint booth, refuel location and large parking area for trucks. Working from this location was a portable plant for making asphalt that was put on the highways. In Garden City, TX the company maintained a 24 hour quarry operation with a crusher. Product was excavated then run through the crusher and moved to a stockpile. Driver operations were provided with Safety Meetings to keep them up to date on any issues of the work underway. Driving records were maintained, accidents were investigated as they occurred, workers compensation claims inspected and any information required was passed to the third party vendors that managed the larger claims. The quarry equipment was inspected daily at any location is might be working. The asphalt plant operations and the fleet working from that plant was reviewed daily.

Reason for leaving: The owner changed Safety Managers on an annual basis, at insurance renewal time.

I worked in the Wichita, KS office as a new employee. I attended three two week sessions of training at the OSHA Training Institute in Des Plaines, IL. At the office I answered phone inquiry from persons regarding regulations concerning a wide range of topics. All information was provided from the 29 CFR 1910 & 1926. I was partnered with one of the senior members and inspections were made in the area of responsibility. I became one person that was sent on all fatal incidents due to my criminal investigation background. One case involved the fatality of two men who were Hispanic. I was able to speak to the only witness who did not speak English. The information developed was provided to a large team who came out to finish the case. For this I was given a reward in front of my peers.

Reason for Leaving: RIF, I was laid off due to more staff than work available.

I was selected after a selection process due to the fact that insurance rates were going sky high in the present. The City following other municipal agencies decided to take action. I was employed and directed to protect the City employees, the government as a whole and the general public from actions of the City. A ten year research (two boxes) of historical information was completed showing a loss history and the lack of an action plan for safety. A review of work in the public developed a needs list added to a survey from department managers. The first program was the down time of employees waiting in the early hours for equipment to be made ready due to repairs. An inspection list was developed and given to the work groups so the drivers would inspect and identify each piece of equipment at the start and end of day. The program was very successful to the point there was no lost time by equipment/employees. Using the needs list training started for all the outside work groups, PPE, good tools, good barricading as needed, heat stress, cold weather and may other topics to improve the quality of the employee. Classes were brought in by outside sources to certify employee groups. No one was left out. A bid program was developed for the next fiscal year. This bid assisted the city in going to insurance companies rather than agency business. Losses went down as the training continued. Workers Compensation cost went down even with the same number of injury incidents. Some programs are in place still. All claims of loss passed across my desk for conclusion. Third party administrators were used as necessary.

Reason for leaving: A job opportunity allowed me to move closer to my family with my young son.

Other activity

Not listed were times that I did contract work for vendors inspecting commercial properties in the Austin, TX area. Consultant work was completed assisting citizens in claims management for loss occurrences. I continued landscaping yard work, help with the needy at my church, worked in a soup kitchen near the University of Texas, offered Drug and Alcohol counsel to some.
