Chemical Dependence Clinician, The Saint Marks Institute for Mental Health
(Unitas), New York, NY As full-time substance abuse counselor and part of
an interdisciplinary team of physicians, mental health counselors and
administrators in the chemical dependency unit I carry a full patient
caseload of 26. I conduct intakes, individual and group counseling,
complete comprehensive bio- psychosocial evaluations, develop treatment
plans, treatment plan reviews, and individual and group progress notes,
prepare referrals, discharges, and coordinate with housing caseworkers,
medical staff and office of parole. 7/13-
Case Manager, The Institute for Community Living, Brooklyn, NY. Under the
general supervision of the Program Director I functioned as part of a team
assigned to support consumers living in independent apartments in the
community. I assisted assigned individuals in developing and maintaining
viable living, working and social situations in the community by obtaining
needed medical, social, psychosocial, educational, financial, vocational
and other services. I participated in the provision of crisis intervention
services to participants. I traveled to/visit consumers' residences or
apartments, provided counsel, assistance and help the consumers, within
their capabilities and interests, in maintaining the greatest degree of
independent living. The majority of duties are often performed

Substance Abuse Clinician, The Addiction Institute of St. Luke's and
Roosevelt Hospitals, New York, NY . First as an intern and later as a per
diem substance abuse counselor at the Inpatient Rehabilitation Unit, I
carried a patient caseload, did individual and group counseling, gave
lectures and attended rounds with the interdisciplinary team of physicians,
nurses and counselors. I reported directly to the Clinical Director and
Senior Counselor. In a timely manner, I completed all necessary paper work
including treatment plans, treatment plan updates, hospital progress notes
and all documents required by New York State Office of Alcohol and
Substance Abuse Services. In the last 3 months at The Addiction Institute
I interned at the Opioid Treatment Program where I did assessments, had my
own patient case load, supervised periodic drug tests and conducted
