Injury patients. Duties include, examining, treating and supervising therapy and rehabilitation for the patients.

Reading x-rays and MRI’s and reviewing and explaining them to patients. Determining necessity for referrals and

evaluating patient progress throughout duration of care. Preparing final narrative reports with Permanent

Impairment Ratings based on the AMA Guides, 6th edition.

2002 – 11/2012 - Florida Department of Health, Palm Beach, Hillsborough and Pasco County Health

Departments. Duties have included Disease Intervention Specialist (Investigating STD's), Environmental

Health Specialist (Inspecting business establishments for biomedical waste and Medical Quality Assurance)

and Health Education and Community Health (developed programs to start SWAT - Students Working

Against Tobacco in Middle and High Schools, in addition to lecturing at hospitals, churches and colleges on

the dangers of tobacco and nicotine addiction.

During the period of employment with the Health Department, I continued treating chiropractic patients at

Abbey Medical Clinic, in Tampa, and Advanced Healthcare Alternatives in New Port Richey, FL.

Prior to 2002 - President, Healthcare West Medical & Rehabilitation Center, Inc., Royal Palm Beach,

Florida In addition to providing chiropractic care to patients, initiated a multidisciplinary clinic, including a

medical doctor, physical therapist, massage therapist, nutritionist andacupuncturist. Team Doctor for Twin
