• Supervisor for established program (“Living Well”) which provides assistive care
services for residents of senior living community (staff of 5+ CNAs/HHAs).
• Provided medication management for clients, conducted twice-weekly blood
pressure clinics, and once-monthly health topic programs for all residents.
• Scheduled services, promoted and maintained program for up to 35 residents.

• On-going evaluations of current clients to assess for increased needs for referral to
skilled services if warrented.
• Worked closely with community’s Wellness Director to identify residents with

BAYADA Home Health acquired Watermark at Home (03/2014) and added its resources
to current program. Duties were expanded to include those of Client Service Manager
(staffing, hiring, payroll, etc.) as well as LPN duties. Nursing duties overseen by RN
(Clinical Manager) and program re-structured to meet new company’s standards.
Position required flexibility to navigate changes from one home health agency to another
with minimum effect to clients established services and staff.

or no supervision on a regular basis.
