• Board of Directors Self-Assessment Survey (2007-2011) Annual survey intended to give Board

Members insight into how to make the Board of Directors more effective. Partnered with General

Counsel to create and administer survey. Analyzed results and produced written report presented to

the CEO and Board of Directors.

• Resource Center Survey; Legal Department Survey (2008, 2009, 2010) Partnered with

Department Vice Presidents to create and administer internal customer-service surveys. Analyzed

results and created reports delivered to CEO, Executive Team and department heads. Facilitated

feedback sessions.

• Team Member Survey (2008, 2010, 2011) Partnered with Human Resources to create and

administer Team Member Satisfaction Survey. Analyzed results and created report presented to the

CEO and Executive Team.

• Manager and Director Leadership Competency Model (2008-2010) Responsible for entire

modeling process including organizing, facilitating and training team; interviewing high performers,

coding and analyzing interviews; developing draft competency model including competencies and

definitions; facilitating a Competency Modeling Charrette for the Senior Executive Team; and

incorporating executive input to create final Leadership Competency Model. This model is currently in

active use within the organization.

administrator Team Member Engagement Survey. Analyzed data and created reports delivered to

CEO and Leadership Team. Was able to examine business metrics, statistically identify major factors

driving Team Member Satisfaction, and identify a direct link between Team Member Satisfaction and

productivity.

• Leaders Pulse Survey (2008-2009) Partnered with CEO to create and administer pulse survey to

management team to determine attitudes and perceptions regarding organizational culture and

challenges. Results of this survey influenced Large-Scale Culture Change strategy and objectives.

• Surgical Center Administrator Competency Model (2008-2009)

• Administrator 360 Survey (2009) Created and implemented custom 360 survey instrument

based upon Administrator Competency Model.
