Case Manager

Description

Unique opportunity for a case manager to assess behavioral health services and other supports for referred inmates from Ohio DRC prisons prior to their scheduled release date. The Case Manager will meet with inmates in the prison setting and ensure that they have transportation upon release, a place to say, and will provide a warm handoff to Southeast and other community partners, ensuring that the individual served is able to access the full range of services as quickly as possible.

Position Requirements

AA/BA required; LSW with Criminal Justice experience preferred.
