JOB SUMMARY

Provide case management to families and individuals that include intake and assessment, specific service deliveries, as applicable, and referral to community organizations and/or component services within the Program for public assistance, health care, employment and supportive services.

ROLE AND RESPONSIBILITIES

Opening, maintaining and closing case files

Completing case notes for all client interactions.

Conduct intake and assessment for clients and family members including facilitation of development of Family Self Sufficiency Plans and Individual Employability Plans with clients.

Assist clients in determining family budget, employment/financial planning, immigration/documentation needs, education and training needs (including providing aptitude/skills assessments), English language training needs, childcare needs, and transportation and interpretation needs.

Maintain regular contact with clients throughout the resettlement, Match Grant and employment service periods, at the following intervals:

Month 1 of service: weekly face to face contact.

Months 2, 3, and 4 of service: monthly face to face contact.

Month 5 of service and thereafter: monthly contacts face to face, by phone or email.

Complete and maintain records pertaining to Resettlement, Match Grant, Employment Services, and/or Outreach and Referral, as applicable.

Provide clients with information, referral and outreach services including distribution of the American Red Cross Resource guide.

Assist clients with individual job search activities and instructions such as creating resumes, completing applications, etc.

Work in collaboration with Job Developers to match clients with appropriate, available job opportunities or placements.

Refer clients to the Immigration Law Clinic for services pertaining to legal immigration documents as needed for employment.

Provide employment retention services including contacting clients at 90 and 180 days after employment and reducing employment barriers such as transportation and translation.

Coordinate service delivery by assisting clients to enroll in services both within the program and with community partners.

Assist clients to enroll in vocational or approved recertification training programs.

Act as liaison between the client and the Department of Human Services (DHS) and the Ingham County Health Department (ICHD), as applicable.

Maintain on-going communication with other Case Managers, Job Developers, Job Trainers, ESL Teachers and other program staff to ensure client needs and requests are addressed. Meet with Job Developers bi-weekly at a minimum to coordinate job search efforts.

Proactively assist clients in meeting all their needs, including adapting to the U.S. and acting as a cultural broker as necessary.

Maintain a professional appearance and behavior as a representative of the Agency and Program.

Discharge duties using independent judgment and discretion.

Attend and actively participate in Agency and Program meetings, conferences, in-services and other meetings deemed appropriate.

Conduct peer review of case notes, if applicable.

Provide interpretation and translation services, both within the Agency/Program and to outside the Agency partners, if applicable.

Provide transportation, as deemed necessary, from the airport (arrival), for D.H.S., Social Security, health appointments, clothing, employment activities, supportive services, etc.
