Read Nurses Log Daily
Monitor Individuals daily for any change in health status
Administer medication at designated times
Review any Incident Reports, Medical Service Sheet, and Health Concerns Forms (in the nurse’s box daily at each Home)
Communicate with the Primary Care Nurse, QIDP and Residential Managers of any concerns.
Sign In/Out when going to each Home.
Assist Physician as needed
Medication:
Review physician’s order sheet, note and transcribe orders.
Review MARS both for oral medications and creams and lotions.
Check the cycle meds against MARS and POS.
Order medication and new orders call: Omnicare Care @ 1-800-435-9555
For over the counter medications to be started the same day you can be picked up.
Update emergency medications periodically (make sure each Individual has 5 day supply).
Review and take the cycle meds to the Home before the end of the month, place in charts.
Review Medical Appointment Sheets for sedation
Weekly:
Daily documentation on MARS
Weekly Nursing Notes on each Individual health status
Check medical supplies
Make medical appointments (sign off, check for follow-up appointment in charts)
Use reinforcements and keep charts orderly
Monitor Acute Illnesses for resolution
Call in new orders and Prescriptions
Check adaptive equipment weekly (helmets, walkers, gait belts, wheelchairs etc.)
Monitor Bowel Charts for daily documentation
Check lab schedules and prepare the forms
Prepare referral slip for those who need specialty appointments
Follow up on lab results, x-ray, mammogram, etc. have Physician review them and file it in the medical book
Give weekend report to the Primary Care Nurse
Assist Physician and attend medical appointments as needed
Monthly:
Vital signs (monthly weights as scheduled)
Submit monthly supply orders to Primary Care Nurse
Send discontinued medications back to Nursing Office to be reviewed by DON
Put monthly medications in medication box and keep organized
Assist with in-service training for staff
Check for special treatment
Monitor Menstrual Charts for consistent documentation
Place MARS and Physician order sheet in RN box for signature and then file
Attend Monthly Nursing Meeting with DON and Primary Care NUrse
Meet with QIDP and/or Residential Managers, check medical appointments, follow-ups, labs, x-rays are done on time
Check creams and lotions for documentation/expiration/refill/re-order date
Check first aid supply including (PRN,Pepto-Bismol, Tylenol, Milk of Magnesia, etc.)
Documentation:
Write weekly nursing note for all People/Persons including weight, blood pressure, pulse and temperature
Write readmission nursing notes for all readmitted People/Persons after hospitalization
Take temperature 1x daily for those on antibiotics as directed
Follow-up on discharge recommendations from ER
Candidates must possess the following qualifications: Valid District of Columbia LPN License Minimum one (1) year of experience as an LPN Must be able to lift weights of at least sixty (50) pounds Strong written and verbal communication in the English language Excellent organization skills and attention to detail with the ability to prioritize and meet deadlines Possess strong ability to think critically, make decisions, and maintain confidentiality Ability to collaborate and work in a team environment
Candidates must provide proof of valid LPN license in DC and CPR and First Aid certifications. All candidates will be required to pass a drug test and Physical
