Job Description Summary: Performs patient care by assessing the patient's physical and functional abilities.

Based on problems determined, the occupational therapist establishes and carries out a plan of care which includes patients/family instruction, d/c planning, documentation in the medical record, communication with the team, and reassessment.

Helps patients to achieve their maximum level of functioning.

Clinical instructor for students, and participates in departmental functions, i.e., process improvement.

Minimum Qualifications: Bachelor's Degree (Required)BLS - Basic Life Support - American Heart AssociationAmerican Heart Association, DL-HC - Driver's License only if in a Home Care department - Department of Motor VehiclesDepartment of Motor Vehicles, LIC - Licensed in Field - State of OhioState of Ohio, OT - Occupational Therapist - American Occupational Therapy Assoc.American Occupational Therapy Assoc.

State Driver's License.

Occupational Therapy.

Licensed in the State of Ohio.

New graduates with excellent references considered.
