Are you passionate about providing comprehensive healthcare support to patients? Cherry County Hospital and Clinic is seeking a dedicated Chronic Care Coordinator to join our team. In this role, you'll be responsible for managing all aspects of a patient’s healthcare maintenance and treatment, ensuring optimal outcomes and facilitating continuity of care across settings.

Duties and Responsibilities:

- Coordinate and implement clinical care management/transition of care plans.

- Identify and enroll high-risk patients, providing tailored support.

- Assess patient wellness and preferences to develop personalized care plans.

- Monitor adherence to care plans and evaluate effectiveness.

- Collaborate with providers to discuss treatment modalities and ensure positive outcomes.

- Communicate findings and recommendations to achieve targeted outcomes.

- Educate patients about their condition, medication, and self-care.

- Assist patients and families in accessing community resources.

- Participate in incentive programs and continuing education opportunities.

Skills and Abilities Required:

- Excellent communication skills, both verbal and written.

- Compassion and empathy in providing patient care.

- Strong nursing knowledge and decision-making abilities.

- Understanding of legal aspects and confidentiality requirements.

- Knowledge of hospital policies and procedures.

- Interest in professional growth and development.

- Ability to work well with patients, families, and healthcare teams.

Minimum Job Requirements:

- Graduate from an accredited school of nursing.

- Registered Nurse license in the State of Nebraska.

- Three or more years of RN experience required.

Physical Demands and Working Conditions:

- Physically and emotionally demanding work.

- Clean, favorable work environment.

- Regular working hours with possibility of overtime.

- Dexterity for operating equipment and performing patient care tasks.

- Potential exposure to hazardous materials; compliance with universal precautions required.
