Job Title: Physical Therapist Department: Ambulatory Services Sports Medicine Physical Therapy Crane The Rehab Team Member works under the general supervision of the Director of Rehabilitation Services and under direct supervision of their clinic manager.

The Rehab Team Member evaluates and treats patients in accordance with the laws and rules that govern the practice of their profession in the state of Ohio.

Although team members report directly to the clinic manager, they are also responsible to each other, to those they supervise (rehab assistants and athletic trainers) and to other participants included in the patient care activities.

Each Rehabilitation Team Member is first a part of a group effort to provide care within a continuous performance improvement environment, and secondly a distinct professional who provides specific contributions to the team approach.

Team members assess patients and establish together plans of care; provide a wide variety of treatment techniques; provide education and support to families; and facilitate continuing care planning.

Team members evaluate quality of services and participate in outcomes management activities.

MINIMUM REQUIRED QUALIFICATIONS Doctor of Physical Therapy (DPT) degree from an accredited school or equivalent combination of education or experience.

Currently licensed in the state of Ohio as a Physical Therapist.

0 years of relevant experience required.

0-2 years of relevant experience preferred.

Requires Candidate to be dual credentialed with a current license to practice Athletic Training and/or currently have their Sports Certification Specialty from the American Board of Physical Therapy Specialties Additional Information: Requires Candidate to be dual credentialed with a current license to practice Athletic Training and/or currently have their Sports Certification Specialty from the American Board of Physical Therapy Specialties Location: Recreation and Physical Activity Center (0246) Position Type: Regular Scheduled Hours: 40 Shift: Varying Shifts Final candidates are subject to successful completion of a background check.
