Hospice Aide
Reports to: Patient Care Manager
Status: Full-time 8:30am – 5:00pm; includes occasional weekend and on-call rotation for holiday and/or PRN
Hours: Monday – Friday
Salary: Commensurate with work experience
Job Summary
The position will provide personal care service to the terminally ill patient in the home or facility that patient resides. The Hospice Aide with provide care in accordance with the plan of care as assigned and delegated by the Registered Nurse and/or Patient Care Manager. Prior to providing care independently to patients, a registered nurse will observe and approve personal care aides delivering care to patients and complete an initial competency evaluation for all personal care tasks assigned to the aide.
Primary Duties and Responsibilities
Provide personal care to the patient in a compassionate and understanding manner consistent with the hospice philosophy
Properly following infection control guidelines with Donning and Doffing PPE and proper handwashing.
Under the written and/or verbal direction of the RN, the HHA performs the following duties:
Provide bed bath, shower or tub bath as appropriate
Provide hair care to include shampooing in the shower or bed bath
Provide appropriate care of skin, hair, mouth and feet; nail care for hands and feet will be provided only as directed by the RN Case Manager.
Provide good body alignment, positioning, and range of motion exercises for non-ambulatory patients
Assist patient in and out of bed and with ambulation (including gait belt, sliding board, Hoyer Lift, E-Z Stand) with family or facility staff assistance as indicated
Assist patient with use of bedpan, urinal, commode and bathroom
Assist with routine catheter care
Assist with feeding and dressing patients
Monitor and record temperature, pulse, respirations and blood pressure, as directed by the RN. Significant changes in the patient’s condition will be reported to the RN immediately
Observing and reporting the following changes:
Observing pain
Respiratory changes
Bowel and bladder changes
Behavioral changes
Changes in nutrition, hydration and activity level
Changes in patient skin; pressure areas, redness, swelling, discoloration
Attend patient care conferences as requested by the RN Case Manager
Identify and address comfort care needs within scope of Home Health Aide role and immediately notifying RN Case Manager of changes
Strong communication skills and support for instruction of patient care to family and caregiver
Maintain the dignity of the dying patient and abiding by patients’ rights with providing privacy, respecting patient’s property and maintaining HIPAA
Support the patients and family’s unique spiritual and cultural beliefs
Provide holistic, family-centered care across treatment settings to improve the quality of life
Consult and collaborate with the interdisciplinary team and others involved in the patients care
Provide care consistent with National Hospice and Palliative Care Organization standards of practice for hospice programs
Maintain supplies as assigned
Provide postmortem care as needed
Qualifications
Georgia Certified Nursing Aides with current certification
Hospice experience preferred.
Valid driver’s license. Reliable, insured, automobile for making home visits required
Belief in the hospice concept
Able to successfully pass a criminal fingerprint background check and drug screen.

Work Conditions and Physical Capabilities
Ability to lift and/or turn patients using appropriate techniques.
Ability to walk, sit, stand or climb stairs -- may be for an extended period of time.
May be exposed to various infections, viruses and diseases from patients & specimens in working environment.
May be required to drive for long distances.
Willingness to visit patient & families in facilities (such as hospitals, long-term care centers, assisted living facilities, personal care homes) and personal homes, being mindful that environmental conditions vary.
