Duties/Responsibilities:
- Provide comprehensive care for chronic rheumatologic conditions.
- Conduct outpatient consultations, creating care plans.
- Offer clinician consultations to internal and external entities.
- Collaborate in developing an intensive outpatient care model.
- Supervise and collaborate with advanced practice providers.
Required Skills:
- Knowledge or experience with NCQA PCMH model (preferred).
- Strong interpersonal and communication skills.
- Ability to thrive in a complex, fast-paced environment.
- Proficient in Electronic Medical Records and Microsoft Office Suite.
Education and Experience:
- Medical degree from an accredited university.
- Licensed physician in Georgia.
- Board certified following specialized training.
- American Heart Association BLS certification required.
- Preferred experience in population health management.
