We are seeking a dedicated and empathetic candidate for our Care Navigator position to directly impact patient health with our virtual care.
The Care Navigator will be responsible in chronic care management and/or remote patient monitoring, in partnership with a healthcare system. Successful candidates will bring experience in educating patients on chronic diseases such as hypertension and diabetes. You will need to be organized and agile in addressing patient needs, and thoroughly document any and all interactions.
Skills and Qualifications:
1-4 years of experience primary care practice or chronic care management
The ideal candidate should be comfortable managing patients via phone, video or texting
Has experience working and documenting an EMR (Electronic Medical Record)
High computer literacy, comfortable learning and using digital health portal
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision
Possess excellent time management and organizational skills
Engaging personality with the desire to learn new topics and provide health education
Pays attention to detail for each task
Is friendly, reliable, and detail-oriented
Is comfortable and confident when speaking to patients
Serves as the primary point of contact for telephone patients with a variety of health or illness questions
Is a positive individual and works well in team environments
Fluent in the English and Spanish
Responsibilities
This position will be remote-with some potential in person meetings
Responds promptly by phone when call is received, and completes task in timely fashion
Reviews patient alerts triggered, reaches out to patient, access situation and provides clinical guidance within their scope of practice
Completes all documentation in a timely manner.
Collects subjective data related to the presenting problem and medical history
Communicates patient information to the patient's provider (following protocols)
Utilizes critical thinking and problem solving skills to recognize and match symptoms to protocol, assigning acuity (emergent, urgent, acute and non-acute) and ensuring safe, timely disposition of health related calls
Works scheduled shift, shift times are subject to vary
Can be a combination of days and evenings hours
Work at least 8 hours in a shift
May have weekend hours
