What you’ll do as a Medical Social Worker (MSW) Case Manager: Conducting comprehensive psychosocial assessments of patients and their families to identify their needs, strengths, and resources
Developing and implementing care plans that address the psychosocial needs of patients and their families
Providing counseling and emotional support to patients and their families, including grief and loss counseling
Coordinating community resources and services to meet the needs of patients and their families
Serving as an advocate for patients and their families, helping them to navigate the healthcare system and access needed resources
Participating in interdisciplinary team meetings to develop and review patient care plans
Maintaining accurate and timely documentation of all services provided
Requirements for Medical Social Worker (MSW) Case Manager:
Master's degree in Social Work from an accredited school of social work
Current state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
Minimum of 2 years of experience working in hospice or palliative care
Excellent communication and interpersonal skills
Ability to work effectively as part of an interdisciplinary team
Knowledge of community resources and services that can support the needs of hospice patients and their families
Ability to provide compassionate and empathetic care to patients and their familiesRequirements:
