Experience you'll need: Nurse Practitioner with 1 years of experience seeing geriatric patients, CA licensed, BLS, ACLS, DEA, Bilingual Spanish is required, valid CA driver’s license, personal transportation, good driving record and auto insurance.

You will work collaboratively with an interdisciplinary team to provide comprehensive care and coverage for older adults, enabling them to live the independent lives they deserve.

Key Responsibilities ● Provide direct medical care to PACE participants ● Complete sections of assessments and care planning ● Manage a panel of participants ● Initiate, evaluate, review and change orders for medications and diagnostic studies or treatments with approval ● Assess, initiate and coordinate care on a daily basis under supervision ● Document all participant interactions, assessments, next steps in the EMR within the specified timeframe set by the center ● Make referrals to specialists with approval and request consults as needed ● Perform history and physicals and participant teaching ● Order, interpret and follow up on lab tests and results ● Perform selected procedures
