CTS Health Services is a community mental health services center that specializes in a holistic approach to health and wellness for children, parents, and adults of varying circumstances and mental challenges. We provide one-on-one behavioral interventions for our clients and their support circles to help our clients navigate through life within their community at their highest level.

We are urgently seeking passionate, authentic, and motivated individual trained in trauma-informed care as a Case Manager to provide general leadership, supervision and management over an assigned service area and staff. The Case Manager will serve as a liaison between the direct care workers, State Director, and Executive Team. They will also work closely with the Utilization Management Team in order to coordinate services. The Case Manager must have excellent communication and documentation skills. The Case Manager will possess an understanding of all mental health and developmental services. The Case Manager will play an active part in the interdisciplinary Trauma-Informed Care Team.

This full-time position may require more than 40 hours a week on the job.

Responsibilities of a Case Manager (not limited to):

Must successfully complete all core required trainings and updates as needed.

Work with a team approach with other employees to ensure agency compliance.

Report critical concerns and problems to Supervisor in a timely manner and actively seek Supervisor’s input and assistance to resolving the concern.

Follow chain of command when encountering problems or other areas of need and correct problems requested.

Solicit and cooperate with assistance of all support staff.

Actively participate in an appropriate manner with the decision-making process as “pro-active” team member and cooperate with team’s decision, whether programmatic or clinical.

Provide coverage on-site or as assigned by the needs of the agency.

Attend staff meetings, planned in-service training, on-going training and any other regular or assigned meetings.

Responsible for any equipment assigned in order to perform duties.

Facilitate flow of information.

Ensure Client Rights are being adhered by for all consumers.

Attend weekly and monthly meetings with State Directors, Clinical Director, and supervisor.

Report to supervisor on a weekly basis on progress or regress of team case load.

Collect data and submit for continuing service authorization, maintaining up-to-date service authorization tracker to sustain 100% authorizations on all consumers within team.

Input and upload consumer demographics onto documentation site (OnTarget).

Complete the ongoing revision of the Individual Treatment Plan and ensuring its implementation.

Monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the Treatment Plan.

Consulting with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DJJDP) providers.

Engaging community and natural supports, and including their input in the person-centered planning process.

Coordinate behavioral health services and other interventions for the consumers with other professionals and Child and Family Team members.

Deliver direct care services when necessary to ensure consumer continuity of care.

Other duties as deemed necessary to provide quality and continuity in services provided.

Supervisory responsibilities:

Provide weekly supervisions to assigned staff, must be documented.

Review/Approve all assigned staff’s service notes to ensure they are billable and in accordance with regulatory standards.

Create/Review and assign service schedules for assigned staff.

Provide routine monitoring for services being rendered by team members for quality assurance in accordance with Medicaid Part 132 requirements.

Maintain/Audit assigned staffs’ consumer charts for 100% compliance (minimum 10 per month); develop plan of correction to rectify non-compliance, follow-through with corrections and report on-going non-compliance issues to Compliance Department.

Participate in quarterly peer-review audits.

Monitor service delivery to ensure goals are achieved to meet consumer needs and ensure continuity of care.

Conduct weekly service follow-up/verification calls to consumers/guardian.

Conducting monthly outcome summaries and progress of consumer goals. Complete discharge summaries within 7-days of discharge.

Responsible to ensure that services are provided in a manner, which meets all applicable regulatory licensure and accrediting regulation.

Other duties as deemed necessary to provide quality and continuity in services provided.

Qualifications of a Case Manager:

Bilingual (Spanish) ability required

Master’s Degree in Social Work from an accredited college/university.

Minimum three (3) years documented experience in the delivery of mental health treatment to population served.

Trauma-Informed Care certified

Proficient in Windows/Microsoft Office programs including Word, Excel, email

Knowledge of Electronic Health Recordkeeping.

Provide current proof of applicable licensing and credentialing requirements.

Must have reliable transportation, maintain required level of vehicle insurance and valid driver’s license.
