Overview:
Pay range for this position is $35.50 - $51.87/hour. Actual pay is based on years of licensure.
POSITION SUMMARY: The Coordinator Patient Flow RN is responsible for facilitating patient transfers, utilizing clinical judgement from regional hospitals to the appropriate care setting by conferencing physicians, documenting the process and patient information in the electronic record. Collaborates with the team for proper bed placement and with LifeFlight, AMT, Lifeline, and other ambulance services to aid the sending hospitals in transporting patients. The Coordinator serves as the liaison between the clinical units, patient access services and care management to assure appropriate and efficient placement of patients from OSF and non-OSF entities into and out of acute care beds and across levels of care using bed board and transfer center technology to facilitate flow. Assists non-OSF facilities with appropriate patient placement. The Coordinator will work with other team members assisting with admissions, discharges, transfers, and staffing within the OSF facilities via use of electronic tools and resources while maintaining appropriate staffing matrix.

REQUIRED QUALIFICATIONS:
Education: BSN
Experience: 1 year of patient care experience
Licensure/Certifications: Current state RN license
Current American Heart Association Healthcare Provider BLS CPR
Other skills/knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented.
