Job Description

PURPOSE OF POSITION:

A licensed clinical social worker is needed to perform the tasks required to improve our quality scores for Healthcare Effectiveness Data Information Set (HEDIS®) from the National Committee for Quality Assurance (NCQA). We need a licensed practitioner to perform a billable encounter to meet these measures. Regardless of the primary payor, all patients discharged from the Psychiatric Emergency Room will benefit from this new position. For our insured patients, it will capture six core quality measures.

SUMMARY OF DUTIES:

• Follow-up after Emergency Department Visit for Substance Use (FUA).

• Follow-up After-Emergency Department Visit for Mental Illness (FUM).

• Follow-up after High-Intensity Care for Substance Use Disorder (FUI).

• Follow-up after Emergency Department (ED} Visit for People with Multiple High-Risk Chronic Conditions(FMC).

• Participates in the formulation of standards, policies, and procedures for orientation, supervision, and in-service training of Social Work staff.

• Provide orientation to new staff and update incumbent staff on changes in policy, procedures, or standard 8.

• Knowledge about departmental, facility, network, city, state, federal, and JCAHO standards and regulations

• Monitors staff activities to ensure compliance with all the above, e.g. supervise the activities and documentation of staff

• Ensures that staff documents all interventions and continuing care plans in the patient's medical record in accordance with departmental and hospital policies.

• Collaborates in completing performance evaluations for staff.

• Gather information needed to complete monthly statistical reports for the Hospital.

• Conducts and/or attends individual, unit, departmental, intra/inter-hospital, and community meetings as mandated by the administration.

Minimum Qualifications

For Assignment to Level I

Limited Permit issued by the New York State Education Department (NYSED) in Social Work. Employees working with a Limited Permit must obtain a valid Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) within one (1) year of the date of appointment.

For Assignment to Level II

1. Valid license as an LMSW issued by the NYSED; or

2. Valid license as an LCSW issued by the NYSED.

For Assignment to Level III

1. Valid license as an LCSW issued by the NYSED.

For Assignment to Level IV

1. Valid license as an LCSW issued by the NYSED and one (1) year of supervisory experience utilizing casework, group work, and/or community organization methodologies.

For Assignment to Level V

1. Valid license as an LCSW issued by the NYSED, and two (2) years of supervisory experience, as described above.

Special Note:

All new appointments to Levels III – V require an NYS-issued Licensed Clinical Social Worker (LCSW) license effective October 18, 2021.
