QUALIFICATIONS:

Bachelor’s Degree in Human Services plus 1-3 years supervisory experience in a day treatment, day habilitation or education program for adults with developmental disabilities or some comparable combination of education and experience.

Qualified Intellectual Disabilities Professional (QIDP) as identified in federal regulation 42 CFR 483.430(a).

Strong interpersonal, communication and organizational skills.

Ability to handle multiple priorities and maintain a high level of confidentiality.

Possession of a clean, valid driver’s license.

ESSENTIAL RESPONSIBILITIES:

Responsible for daily operations of Day Habilitation program. Direct supervision of all Day Habilitation staff to include staff discipline and performance evaluations.

Provide direction for program goals and objectives for individuals served.

Responsible for coordination of scheduling of various core and routine services and assure compliance with OPWDD guidelines.

Assure programming is designed to be person-centered by identifying wants, needs, dreams for all individuals in the Day Habilitation Program.

As a QIPD develop Day Habilitation Plans to establish program goals and objectives in compliance with Day Habilitation allowable service.

Required to attend annual LifePlan and SAP review meetings.

Development, schedule and track all staff annual training and ongoing staff development.

Monitor and evaluate program effectiveness through conducting individual satisfaction surveys.

Provide consultation on problems with specific individuals.

Assist in the recruitment and selection of all Day Habilitation program staff.

Assist in the screening and evaluation of referred individuals.

Responsible for ensuring staff utilize Precision Care for all individual program records.

Responsible for record retention and destruction per agency policy and procedure.

Programmatic oversight of Community Inclusion Specialist, Developmental Specialists, Direct Support Professionals (DSPs).

Selection of resources necessary to fulfill programmatic needs. Participate in developing and maintaining program budget.

Expeditiously report any sickness, injury or seizure to medical authorities.

Ensure that all record keeping and documentation completed by program staff is Medicaid compliant.

Responsible for the review of Medicaid documentation to ensure service delivery.

Responsible for the submission of Day Habilitation billing to accounting (review of attendance log, monthly data record, and monthly summary notes.

Participate in other agency committees.

Department related travel as requested.

Keep abreast of current knowledge in the field.

Professionally represent the Agency at all times.

Responsible for the safety of staff and persons served under his/her supervision.

Perform any other related duties as directed by the Director of Day Habilitation Program and/or designee
