Title: Clinical Case Manager (PART TIME) LSW/LPC

Location:2850 S Wabash

Shift: 8:30am to 5pm Monday, Wednesday, Thursday

Under the guidance of our Program Manager, you'll provide crucial clinical case management services, shaping the lives of adults, immigrants, refugees, and those dealing with mental health, HIV/AIDS, or substance abuse issues.

Duties & Responsibilities:

Delivers short-term clinical case management services, creating and monitoring service plans.

Engages in regular meetings with assigned cases and collaborates with community representatives.

Provides individual counseling to address motivational and psychosocial challenges.

Be a lifeline through intense advocacy, crisis intervention, and problem-solving.

Conducts outreach, and intakes, and makes referrals to empower our program participants.

Screens for income support, advocating for benefits on behalf of our community members.

Develops and oversees treatment plans, ensuring realistic goals are achieved.

Extends financial assistance and management support in alignment with our organizational policies.

Cultivates professional relationships with governmental, health, and social service agencies.

Participates in supervision and training sessions, contributing to our collaborative team.

Qualifications:

Master’s degree in Social Work, Counseling, Clinical Psychology, or a related field.

Minimum 3 years of experience in Social Work, Counseling, Clinical Psychology, or a related field.

LPC/LSW license is required.

Valid driver's license.

Knowledge/Skills/Abilities (K/S/A)

Excellent written and verbal communication skills.

Strong customer service and organizational abilities.

Knowledge of local support service providers.

Proficiency in Microsoft Office and relevant software.

Commitment to a strength-based, trauma-informed, and human rights-grounded approach.
