Overview of Duties and Responsibilities:

This is a clinical position providing outreach mental health services to adults within nursing homes and older adults (age 60 and older) regardless of residence type. Interventions include individual and group psychotherapy, assessment, and consultation with professionals and support staff.

Perform related duties as assigned by supervisor

Maintain compliance with all company policies and procedures and applicable rules and regulations of grantors.

Models the Pillars and guiding principles of NSO at all times.

The responsibilities of this position are to provide outpatient psychotherapy and social work monitoring duties in nursing home and clinic settings to adults and older adults in Wayne County who participate in the MiHealth Link pilot program. The position requires a fully licensed LMSW to fill the role. Candidates who do not possess a full LMSW will not be considered.

Potential Monthly bonus Starting at $500!

Driving/Travel RequirementsEducation, Licensure and Experience Requirements:

Must have access to reliable private transportation

Must maintain valid MI driver’s license/State ID

Must maintain automotive insurance

Will be expected to travel on NSO time

Annual Training

Non-Violent Intervention training is required for this position

Additional Trainings will be required

Education: Master’s in Social Work (MSW)

Required Licensure: Licensed Master Social Worker (LMSW) in the State of Michigan

Minimum Required Experience: Course work or hands-on experience with the elderly, or mentally ill persons.

Intermediate Computer Skills; must be able to work in an electronic health charting system.

Job Knowledge and Comprehension and duties

Primary Job Role

Gathers information pertinent to consumers functioning and consistent with best practices of clinical assessment.

Formulates person centered treatment plans which reflect identified needs, desires, services, and measurable goals synthesized from the assessment and person-centered meetings.

Formulates provisional diagnoses consistent with the assessment.

Identifies persons in need of hospitalization or placement into other protected environments.

Appropriately shares findings and recommendations with consumers, significant others, and professionals in order to ensure implementation of treatment plans and integration of services with other agencies/institutions.

Establishes supportive and effective relationships with consumers, care givers, significant others, and professionals.

Productivity

Responds to referrals within the following time frames.

7 Calendar days for persons discharged from emergency psychiatric settings.

14 calendar days for non-emergent intakes

Disposition made within 3 hours for crisis evaluation

Maintains an ongoing caseload of 1.5 standard deviations from the mean Outreach population

Provides individual and/or group intervention to clients according to the amount, duration, and frequency in the consumer’s treatment plan 90% of the time.

Progress notes will be entered within 48 hours 75% of the time.

Performs other duties as assigned.

Working Conditions and Necessary Capabilities

Works in an outreach capacity; uses a computer, telephone and other office equipment as needed, to perform duties. Work location can be in an office-style setting, or at a consumer’s residence or place or respite

The noise level in the work environment is variable, due to frequent contact with consumers.

Bends, stoops, and reaches in order to file, search for, and retrieve records and documents.

Manual dexterity and regular fine-finger and hand/wrist motions are required for operating a keyboard, writing, and filing.

Seeing/vision, talking/speaking and listening/hearing are continuously required.

Frequently required to stand during working hours.

Frequently required to sit, walk, or drive.

Operates an automobile while performing assigned job duties.
