Job Description

Completes medical histories and physical exams and provides treatment to HMSS patients and consultative services to appropriate specialty groups in accordance with: the medical plan of care; critical pathways established by Self Regional Healthcare, Self Medical Group, and Medical/Nursing Policies /Procedures /Protocols and Standards of Care. Completes admission / discharge medication reconciliation, orders and interprets labs and imagining studies, and completes discharge summaries. Works in collaboration with Physicians, Case Management, patients and families. Initiates advance care planning; works with supportive care and hospice teams to assist patient and family with end of life decisions. Performs any other assigned duties per HMSS Physician, VP Of Medical Affairs, and Self Medical Group Administration. Maintains continuing medical education requirement in accordance with hospital requirements; maintains BLS and ACLS certifications. Minimum 1 year clinical Physician Assistant experience required.

1. Takes a systematic approach in assessing the patient's physical, psychological and social status determining the need for care, the type of care to be provided, and the need for reassessment. Determines the scope and intensity of reassessments by the patient's diagnosis, care setting, desire for care, response to previous care, consent to treatment and applicable Standards of Care.

2. Demonstrates ability to do most important work first. Able to manage assignments. If workload is increased, is able to prioritize so that vital activities are accomplished. Assists others without being asked. Present to work on time, engaged to assist patients.

3. Demonstrates ability to perform and/or assist physician/other health care providers with routine procedures within scope of practice/expertise. Assist HMSS physicians with new admissions and floor questions/needs for the direct care of inpatients. Assist with ECC Bridge Orders to ensure patients are seen timely at admission.

4. Delivers care in a nonjudgmental/nondiscriminatory manner that is sensitive to the linguistic, cultural and spiritual needs of the patient, family, and visitors. Achieve SRH Goal of 75 tile% patient satisfaction for inpatient care.

5. Demonstrates good judgment and knowledge to the physicians and staff. Always goes above and beyond to meet the needs of department, patient's and team member's needs. Achieve 6 of 8 domains of SRH HCAHPS goals.

6. Attended 100% of staff meetings since last appraisal or has documented excused absence(s). Completes annual Educode, annual health test requirements & SRH required continuing education annually.

7. Communicates in a clear and concise manner both in oral and written communication to the patient, family member/care giver, fellow team members, physicians, practice manager and SMG Hospital-Based Practices Director.

8. Creates atmosphere of enthusiasm in the office. Makes extra effort to assist team members and management with problems or questions. Is a positive example to others in the department. Promotes cooperation both inside and outside the department. Flexible with work schedule to accommodate patient needs and overall needs of the department.

9. Viewed as an expert in identifying, providing, and directing others in meeting the needs of the population served. Utilizes knowledge of growth and development in communicating and interacting with patients and family members/caregivers. Reduce SRH OSHA Recordables by 10%.

Graduate of an accredited PA program, Certified by the National Commission on Certification of Physician Assistants, NCCPA National Certification Exam completion and passing, current licensure, clinical expertise, leadership, and organizational ability. Must be able to communicate effectively and work collaboratively with others. Must be self-motivated. Must have significant practice experience in private practice or institutional setting.
